An Affidavit form is a sworn statement by a person that is voluntarily under oath. Typically there is a penalty for perjury and this is a type of legal statement. The form must be the truth to the extent or his or her knowledge.

Financial Affidavit in Support of Petition for Waiver of Fees

You need to submit this financial statement showing the breakdown of your assets, liability, income and expenses to support your petition for waiver of  the requirements for you to pay fees.  You sign the  financial statement in the presence of notary public or Clerk of Court.

Alaska Affidavit

This affidavit is a standard one used in the District/Superior Court for the State of Alaska. The affiant states under oath and affirmation that the facts contained in the affidavit are true.  Also, the affiant  needs to subscribe and swear  or affirm before  the Clerk of Court, Notary Public or other person authorized to administer oaths.

Colorado Affidavit to Accompany Application for Marriage License

This No Social Security Number Affidavit must be completed by the applicant. According to the Colorado Revised Statutes 2013 (C.R.S. 14-14- 113(1)(a), “if an applicant for a professional or occupational license, commercial driver’s license, or marriage license submits a sworn statement, together with the application, stating that the applicant does not have a social security number, such applicant shall not be required to provide a social security number on his or her application as required in subparagraph (I) of this paragraph (a). “. Thus, the applicant is making a sworn statement to accompany the application for a Colorado marriage license.

Colorado Financial Affidavit

A Financial Affidavit must be completed by any party accurately who is asking for or being asked to pay child support, children expenses, college expenses, spousal maintenance (alimony) and attorney fees.

Indiana Affidavit

The affiant states under oath and affirmation that the facts contained in the affidavit are true. The completed affidavit is to be submitted to the Bureau of Motor Vehicles  in the county of ________, State of Indiana.

JDF 73T Affidavit-Generic R10-13 (FINAL)

This Generic Affidavit must be signed by the applicant. Also, it must be completed, signed and notarized by the Notary.

Texas Affidavit of Absent Applicant

An applicant applying for a marriage license needs to appear personally before the County Clerk. If unable to appear, this Affidavit is to be completed. On behalf of the absent party, the other designated applicant may apply for a marriage license.
