Name/Address/Office Change Form

The purpose of this form is to notify the US Senate Select Committee on Ethics about the recent change of information of the filer of this form. The notice involves the change of name, address, or office for updating purposes. The Public Financial Disclosure Report of the filer will be updated.

Form 8833, Treaty-Based Return Position Disclosure Under Section 6114 or 7701(b)

This form is to be completed by the taxpayer. The purpose is to make the treaty-based return possible disclosure pursuant to section 6114 of the Internal revenue Code. Any dual citizen taxpayer is required to file this form for the purpose of making the treaty-based return position disclosure pursuant to Regulations section 301.7701(b)

AO 104, Request for Examination of Report Filed by a Judicial Officer or Judicial Employee

The applicant is authorized by section 105 of the Ethics in Government Act of 1978, as amended, to examine report/s filed by Judicial Officials or Judicial Employees. Financial Disclosure Report National Security Positions

This form is about the Privacy Act Notice for Financial Disclosure Report pursuant to Public Law 93-579 (Privacy Act). Providing the information required will help the government agency concerned in conducting national security investigations. The employment of the employee holding any national security position is ensured that it is consistent with the nationals security interests.

California Lead Based Paint Disclosure Form

In this disclosure form, some important details are taken into consideration: (1) Seller’s or  Landlord’s Disclosure; (2) Listing Agents’ Acknowledgment; (3) Buyer’s or Tenant’s Acknowledgment; and  (4) Cooperating Agent’s Acknowledgment, among others. The notification for the purchase by any  individual of  a residential real property is that such property may expose a danger from lead-based paint.

Landlord Lead Based Paint Disclosure Form

If a property was built before 1978 then this form must be given to the lessee to verify that there may be exposure to lead based paint.

Seller Disclosure Lead Based Paint Form

This form provided by the EPA allows a landlord who is selling his or her property to alert the new buyer that there may be lead based paint inside the home. If the house was constructed prior to 1978 most likely this form will have to be signed by all parties.
