inv60, Request for Determination and Advisory

In this  form,  an examining  office or agency (Office of the Personnel  Management or agency)  is  requested to determine whether an applicant is suitable  for Federal employment subject  to  scrutiny based on factors set forth  by them.  If found unsuitable for  Federal employment, many actions such as cancellation of eligibility,  removal,  debarment,  and the like may apply. Likewise, this  form is used to allow another bidder or  applicant who  has  a preference  eligibility to participate based upon conduct.

inv100, Freedom of Information or Privacy Act Record Request Form

The Office of the Personnel  Management-Federal Investigative Services  webpage discusses about this request form.  As a requester, you can request your own  background investigation records  from the Office of the Personnel Management-Federal Investigative Services (OPM-FIS) pursuant to the Freedom Information Act and the Privacy Act. The use of the Freedom of Information/Privacy Act  Record Request form   is not mandatory. However,  you can use any other format provided you make sure that it contains a detailed description of the information  you are requesting including sufficient personal identification data. Following the submission  requirements will help you acquire  your request without any further information, otherwise , no action being taken on your request.

inv100a, Privacy Act Request for Completed Standard Form SF85 or SF85P or SF86

The requester  provides  information  to be used in  locating and  providing responsive records in compliance with the Freedom of Information Act (5 U.S.C. § 552), and/or the Privacy Act of 1974 (5 U.S.C. § 552a). To  submit an accurate privacy act request, the requester  needs to read and understand fully the Privacy Routine Use and  the Disclosure Information (page 2 of the form).

Follow the instructions provided  to you by  the person whose purpose is to assist you in the completion of this form. Background investigations and reinvestigations  being conducted by the U.S. Government using your honest responses on this questionnaire. The purpose is to determine whether or not the applicant or incumbent, either employed by the Government or working for the Government under the terms of the contract, is suitable  for  the job.

Follow the instructions and other clarifying requirements provided  to you by  the person whose purpose is to assist you in the completion of this form. Background investigations and re-investigations are  being conducted by the U.S. Government  using your honest responses on this questionnaire. The purpose is to develop information whether or not you are eligible for the public trust or sensitive position, expected to be  reliable and loyal to the U.S. Government.
