The lease addendum adds features or provisions to the existing lease. The lease is being amended with the integration of the provisions introduced in the lease addendum. The provision of the lease addendum prevails in the event that any conflict arising between both provisions of the lease and of the lease addendum.

HUD-40030 – Claim for Temporary Relocation Expenses (Residential Moves)

Its use is under the supervision of the Office of Community Planning and Development of the U.S. Department of Housing and Urban Development. This is valid until April 30, 2018. Review first the Public Reporting Burden and Privacy Act Statements found at page 3 of the claim form before completing this form. Families and individuals who apply for reimbursement of temporary relocation expenses can use this claim form. The Agency will help the claimant complete the form. If the full amount claimed is not approved, the Agency will provide a written explanation for the disapproval. HUD-40055 – Claim for Moving And Related Expenses – Nonresidential

Review first the Public Reporting Burden and Privacy Act Statements found at page 5 before completing this form. Displaced businesses, nonprofit organizations, and farms can use this claim form. They claim a payment for Actual Reasonable Moving and Related Expenses. Also, they prefer Reestablishment Expenses. They do not want to claim a Fixed Payment. Other details to consider are found at the claim form. The Agency will help the claimant complete the form, with emphasis in explaining the difference between the two payments as indicated in the form. HUD-40057 – Claim for Replacement Housing Payment – 180 Day Homeowner-Occupant

Families and individuals who apply for a replacement housing payment can use this form. This is in consonance with the Uniform Relocation Assistance and Real Property Acquisition Policies Act of 1970 (URA). This also pertains to a 90-day homeowner occupant who elects to buy a replacement home. The use of the form HUD-40058 is also applied to a homeowner-occupant who decides to rent rather than buy. Families and individuals who apply for rental or down payment assistance may use this form. Likewise, this form may be used by a 90-day homeowner-occupant who opts not to buy a replacement home but rather to rent a home The Agency will help the claimant complete the form. If the full amount claimed is not approved, the Agency will provide a written explanation for the disapproval. If not satisfied with the determination of the Agency, the claimant can make an appeal through its assistance.

HUD-40061 – Selection of Most Representative Comparable Replacement Dwelling

Its use is under the supervision of the Office of Community Planning and Development of the U.S. Department of Housing and Urban Development. This is valid until April 30, 2018. For public notice, the collection of this information is to determine the most comparable as well as available replacement housing and its cost. These variables are necessary for Agencies to compute a replacement housing payment for displaced persons. For privacy notice, the purpose of soliciting an information from the claimant is to determine whether the claimant is eligible to receive a payment. The payment sought is needed to help the claimant rent or buy a new home. It is not mandatory to provide this information. However, if the claimant does not furnish the information, the claimant may not receive any payment for the expenses or it may take longer to pay the claimant. The collection of information is in consonance with Uniform Relocation Assistance and Real Property Acquisition Policies Act of 1970. The Federal agency may review for any information collected through this form.

HUD-40072 – Claim for Rental or Purchase Assistance (sec. 104(d))

Its use is under the supervision of the Office of Community Planning and Development of the U.S. Department of Housing and Urban Development. This is valid until April 30, 2018. The purpose of soliciting an information from the claimant is to determine whether the claimant is eligible to receive a payment. The payment sought is needed to help the claimant rent or buy a new home including the amount of any payment. The Agency will help the claimant complete this claim form. If the full amount claimed is not approved, the Agency will provide a written explanation for the disapproval. If not satisfied with the determination of the Agency, the claimant can make an appeal. It is not mandatory to provide this information. However, if the claimant does not furnish the information, the claimant may not receive any payment for the expenses. The collection of information is in consonance with Section 4(d) of Housing and Commission Development of 1974, as amended. The Federal agency may review for any information collected through this form.

This lease is made and entered into between Ryan Companies US, INC. as the Lessor and the United States of America as the Government. The property being leased is categorically described in item 1 of the lease. Both parties are expected to follow the terms and conditions stipulated in the lease.
